Doctor appointment booking system: what it does and what it costs in 2026
A doctor appointment booking system lets patients pick a doctor and a slot (or an OPD token) online, confirms the booking on WhatsApp or SMS, reminds them before the visit, and passes their details to reception and the prescription pad. In India, paid clinic tools start at ₹999/month (Practo Ray, billed yearly) and go up to ₹1,00,000/year for Eka Care's 5-doctor plan. Zoho Bookings, a general scheduler, has a free plan.

How booking works: the patient journey in 6 steps
- Patient opens your booking link from Google Maps, your website, Instagram, WhatsApp or a QR code at the counter.
- Picks a doctor and a free slot, or takes a token for a walk-in session. Optional questions (reason for visit, new or follow-up) speed up reception.
- Gets an instant WhatsApp confirmation with doctor, time, map pin and any deposit paid.
- Gets reminders 24 hours and 2 hours before, with confirm, reschedule and cancel buttons.
- Checks in at reception and joins one token list with the walk-ins, with a "you're next" message.
- Sees the doctor, who opens the record created at booking. The prescription goes out on WhatsApp and the follow-up is booked before the patient leaves.
Ask each vendor to show you steps 1 to 6 without reception re-typing anything; every re-typed name brings back errors and phone calls.

One booking link everywhere patients look
Test your booking link on a basic Android phone on mobile data. If picking a slot takes more than three taps, patients will call instead.
Patients without a smartphone still phone or walk in. Reception books them into the same system, and the confirmation goes by SMS or to a family member's WhatsApp.
Walk-in tokens and booked slots in one queue
Arguments start when a 10:00 walk-in watches a booked patient who arrived at 10:40 go in first. Put the rule on the wall:
- Keep a fixed share of each hour for walk-ins, for example every third 10-minute slot.
- Give booked patients a grace window, say 10 minutes, after which they join the walk-in line.
- Run one token display for both, and send a WhatsApp "two patients ahead of you" message so people can wait outside.
WhatsApp confirmations and reminders with one-tap reschedule
Use utility templates with confirm, reschedule and cancel buttons, in English plus Hindi or your regional language. The reschedule button matters most: a patient who can't come releases the slot instead of simply not turning up. When a slot frees up, the system should offer it to waitlisted patients on WhatsApp.
A 24-hour reminder can be this short:

Patients must opt in to WhatsApp messages, which is easiest as a tick-box on the booking page. Consent wording and DPDP points are covered in our WhatsApp automation guide for doctors and clinics.
Advance payment via UPI, and when not to charge
A small UPI deposit filters out "just in case" bookings. The trade-offs: some patients drop off at the payment step, refunds take staff time, and gateways charge a fee (Practo Ray lists 2% or 1.8% per transaction, depending on plan).
- Charge it for: first specialist visits, long procedure slots, evening and weekend prime slots.
- Don't charge it for: follow-ups, elderly patients, urgent cases and regulars.
- Keep it small: a fixed amount well below your fee, adjusted against the bill and fully refunded if the patient cancels before a cut-off, such as 3 hours before.
- Test it first: switch it on for one doctor's first-visit slots for a month and compare bookings and no-shows with the month before.
Multi-doctor calendars, leave blocking and branches
Each doctor needs their own OPD days, slot length (a 10-minute review is not a 20-minute new consultation), breaks and surgery days. Patients should choose a doctor or "first available". Blocking leave should automatically message everyone already booked with a reschedule link. Each branch needs its own calendar and map pin.
If you also take marketplace bookings, ask whether they arrive in the same calendar. If they don't, give the marketplace its own fixed slots so the same 11:30 is never sold twice.
Teleconsult links sent automatically
Eka Care's plans include teleconsultation (200 free minutes on Eka Doc Plus). Zoho Bookings connects with Google Meet, Zoom, Zoho Meeting and Microsoft Teams. Practo Ray's plans page doesn't list it. Our software doesn't include it built in; we add it as a custom module on request.
Hand-off to patient records, and who can see them
The booking should create or match the patient record by mobile number, so reception never types the name twice. In India one number often belongs to a whole family, so the system must hold several patients under it.
Ask where data is hosted, who sees what, and how backups work. Our SaaS version encrypts data, keeps daily backups and uses role-based access.
ABHA Scan and Share: what it is and when a private clinic needs it
- What it is: launched in October 2022 under the Ayushman Bharat Digital Mission. A patient scans the facility's QR code at the OPD counter to register and get a token without paperwork. As of June 2024, the National Health Authority reported over 3 crore OPD tokens across 5,435+ facilities in 35 States/UTs, with Uttar Pradesh leading at about 93 lakh.
- Who needs it early: walk-in-heavy OPDs where patients already carry ABHA. Mostly booked clinics can make it phase two (see the FAQ on whether it is compulsory).
- What integration costs: our ABHA integration guide estimates M1 integration at ₹1.5L–₹3L plus ₹50K–₹2L for the Safe-to-Host audit, over 4–8 weeks.
Do WhatsApp reminders cut no-shows, and what does each one cost?
I won't quote a percentage; vendor no-show figures are not independent research. In our builds, reminders help most when they carry a reschedule button, because the patient who can't make it frees the slot for someone else.
Utility vs marketing templates. Confirmations, reminders and follow-up notices are utility messages. Offers, health camps and festival greetings are marketing messages and cost far more. Meta assigns the category when it approves a template, so keep reminders strictly transactional. The cadence I recommend: a confirmation at booking, then reminders 24 hours and 2 hours before.
Worked example. Assumptions: 30 booked appointments a day, 26 OPD days a month, 3 messages per appointment. That is 780 appointments and 2,340 messages a month.

Add-on, not a booking tool: Practo Ray Connect is a credit-based call assistant. Connect 1000 costs ₹999–₹1,499/month and Connect 4000 ₹1,999–₹2,499/month, depending on term. Calls use 1 credit per incoming minute and 2 per outgoing minute; unused monthly credits lapse.
Notes on the 3-year column:

How I'd choose: Practo Ray's ₹999 plan is the cheapest verified paid clinic plan here and suits a doctor already getting patients from Practo.com, though it lists SMS, not WhatsApp. Eka suits a single doctor who wants teleconsult built in. Setup checklist: go live with online appointments in 7 days
Before day 1: record your baseline
For one normal week, count no-shows per session and incoming phone calls per day on a sheet at reception. Without these numbers you won't know whether anything improved.
Day 1–2: doctor schedules, slot lengths, token rules and holidays
For each doctor, write down OPD days and hours, slot lengths for new and follow-up patients, breaks, online bookings allowed per session and the walk-in share. Add holidays, known leave, fees and your deposit rule.
Day 3: WhatsApp number and utility templates
Use a clinic number, not the doctor's personal phone, and start Meta business verification early; it is often the slowest step. Want to keep the WhatsApp Business app number patients already have? Ask your provider whether it can move to the API and whether the app keeps working on it; options differ. Prepare five templates in English and one regional language: confirmation, 24-hour reminder, 2-hour reminder, reschedule or cancel, and follow-up due.
Day 4: booking link on Google and at the counter
Paste the link into the appointment or booking link field of your Google Business Profile, then add it to your website, Instagram bio and WhatsApp Business profile. Print a QR standee for reception and put the QR on your prescription pad.
Day 5: receptionist training and patient import
Plan one hands-on session of an hour or two with your real schedule loaded; ask whether the vendor includes training. Give reception two lines. On the phone: "I'm sending you a booking link; you'll get a WhatsApp confirmation." For walk-ins: take the mobile number and issue a token in the system, never on paper. Import your patient list (name, mobile, last visit) so repeat patients are recognised on day one.
Day 6–7: soft launch with one doctor, then measure
Go live with one doctor. Each week, compare against your baseline: no-shows by doctor, bookings by source (Google, website, marketplace, WhatsApp, walk-in, phone), call volume and waiting time. Fix confusing template wording, then add the other doctors and hold a short second training session.

7 mistakes that keep your clinic's phone line jammed
These are the patterns we see most often in the booking systems we've built:
- English-only reminders. Patients call to check what they say.
- No reschedule button. A patient who can't come just doesn't come.
- A deposit on every visit. Follow-ups and elderly patients drop off.
- Doctor leave not blocked. Patients find a closed cabin and leave a bad review.
- A separate walk-in register. Two queues guarantee double-booking.
- Only the marketplace. Repeat patients keep booking through someone else's app.
- No data-export clause. Your patient history gets stuck when you switch.

Most cloud booking systems run in a phone browser, so open any demo on your own phone before you buy; ours works on phones, tablets and desktops. If the connection drops, issue tokens from that list and enter them later.
